Adults (>8 yrs): 2 ml/kg of D50 2-8 yrs: 4ml/kg of D25 <2 yrs: 10ml/kg D10 Note: 1 amp = 50 ml D50 = 50g dextrose/100 ml--> 25g of suga

Hydrocortisone
A
Adults: 100 mg IV

Insulin (DKA)
A
Bolus: 0.1U/kg (usually 10U) IV [boluses not typical in kiddos]
Drip: 0.1 U/Kg/hr

- 0.5-2 mg ( usually start with 0.1-0.2 mg, pre-treat with anti-emetic)
- 0.01 mg/kg initial, repeat at 0.1 mg/kg prn respiratory depression

Lidocaine
A
For stable VT or persistent pulseless VT/V fib after amiodarone or if amiodarone unavailable
1-1.5 mg/kg initial bolus, then 0.5-0.75 mg/kg q5 min to max dose of 3 mg/kg

“LEAN” (lidocaine, epinephrine, atropine, narcan)
- Double the dose and mix in 10 ml NS
