Characteristics of good CPR
Chest compressions:
100-120 per min
Depth 2 inch
Allow for chest recoil
Minimize interruptions
Frequency of breath when with advanced airway and without airway
With airway: No interruption, 1 breath every 6 seconds
Without airway: 30:2 compression ventilation ratio
ROSC
Return of spontaneous circulation
Meds for VF or Pulseless VTach
Epinephrine 1 mg q3-5 minutes (increase myocardial and cerebral flow)
Amiodarone 300 mg x1 then 150 mg x1
OR
Lidocaine 1-1.5 mg/kg x 1 then 0.5-0.75 mg/kg afterwards
To consider when torsade de pointe
Magnesium
Polymorphic VT associated with long QT interval
What are Asystole/PEA Meds
Epinephrine 1 mg q3-5 minutes
What are bradycardia Meds
Atropine 0.5 mg IV q3-5 minutes (max 3 mg)
Should NOT delay external pacing
What are the signs of instability?
1- Chest pain
2- Dyspnea
3- Hypotension
4- Altered mental status
Meds for Narrow Complex Tachycardia
If PSVT does NOT respond to vagal maneuvers
Adenosine 6 mg IV push, wait 2 minutes then repeat 12 mg IV
How much time should be given at maximum for checking a pulse?
10 seconds
Compression-ventilation for adults versus children
Adults: 30:2
Children: 15:2
Depth of compression for children
At least one third AP diameter of the chest
