Aircraft collided with another on the runway when they were diverted to another airport. The aircraft on ground was not cleared for takeoff and there was another aircraft on the runway.
How: more emphasis on CRM

Pilots went below minimums before the runway was in sight when the ils was out
How: pilots aren’t allowed to descent below mins

Boeing 727 overtook a Cessna on final and the two collided. There was no tower advisory for the sake of the Cessna.

Engine fell off on takeoff due to poor maintenance practices
How: improved maintenance standards and flaps lock in place when hydraulic pressure is lost
