What is a hazard?
A workplace condition that might result in injury, illness, disease or death or might result in damage or loss of property or equipment
What is a mishap?
Any unplanned or unexpected event causing death, injury, illness or material loss or damage.
What is the difference between hazard and mishap?
Hazard is condition that is expected while mishap is not.
What is a class A mishap?
- Any mishap resulting in cost of damage to govt of $2.5 million or more.
- DOD airplane is destroyed.
- Injury or occupational illness that results in a fatality or permanent total disability.
What is a class B mishap?
- Any mishap resulting in cost of damage to the government of $600,000 but less than $2.5 million.
- Injury or ilness that results in perment partial disability.
- When 3 or more personnel are hospital for inpatient care as a result of a single mishap.
What is a class C mishap?
- Any mishap resulting in cost of damage to the government of $60,000 but less than $600,000
- Nonfatal injury or illness that results or 1 or more days away from work, not including the day of injury.
What is a class D mishap?
- The resulting total cost of property damage is $25,000 or more, but less than $60,000.
- Recordable injury that is not beyond first aid.
When is the first day that counts as a loss work day?
- The day after.
- Day of, does not count
Safety Investigation Report (SIREP) must be submitted to the Naval Safety Center via Risk Management Information (RMI) within how many days by safety?
30 days after the local mishap investigation.
What is the MDR responsible for as it pertains to A&I?
Submit the injury report, and treating the injury.
What is a HAZREP?
Hazard Report it is submitted for near-mishaps, not required but encouraged as a teaching point.
What class mishap is a Safety Investigation Board required (SIB)?
Class A
