What kind of mishaps require a investigation or report?
- Lost work day
- All fatalities that occur within 1 hour of command PT
- Any medical diagnosed work related injurie such as cumulative trauma or MSK
- Threshold shifts
- Work related needle stick injury or cut from a sharp object that is contaminated with potentially infectious material
What is a Class A mishap
- Damage resulting in more than 2mil or more.
- Fatality or permanent total disability
- aircraft destroyed
Class B mishap
- 500,000 or more to less than 2mil
- Permanent partial disability
- when 3 or more are inpatient for a single mishap
Class C mishap
- 50,000- less than 500,000
- Nonfatal injury resulting in 1 or more days away from work not including day of injury
Class D Mishap
20,000 or more but less than 50,000
any injury not covered by any other class
