decompensated heart failure 

If they have these-cancel surgery unless it’s emergent

Ischemic Heart disease: 
history of MI
positive stress
use of NTG
complaints of chest pain 2/2 coronary ischemia 
EKG with abnromal Q waves 
CHF 
pulmonary edema 
paroxysmal nocturnal dyspnea 
peripheral edema 
bilateral rales 
s3 heart sound 
Cerebrovascular disease 
Hx of TIA 
Hx of CVA 
preoperative insulin tx fro DM 
Preop creatinine higher than 2 mg/dL 

Take type of surgery into consideration as well
