Report to staff dr of service chief.

What to include in the abuse report
Name, address, age of child.
Name and address of abuser.
Facts of abuse.
Signs of abuse
Similar to trauma. Does it make sense? Document in patients own words. Look for other signs- lots of bruises? Document always.
Signs of abuse. Indirect injuries
Subconj heme- choking, coughing.
Hemorrhages in and around eye. Can see in posterior pole/all layers of retina. Can see retinal exudates- Purtscher’s retinopathy.
Bruises in accidental and intentional locations
Accidental- knees/elbow, chin/forehead, cheek bones, lower legs and lower arms.
Intentional- Upper arm, thighs, chest, abdomen, face, ears, neck
Differential diagnosis of retinal hemorrhages in children
Birth related- up to 59% with prolonged labor. or ROP.
Leukemia 10% ALL Sickle cell Bleeding disorders Meningitis HTN
Shaken baby syndrome (SBS)
Rental hemorrhage and intracranial hemorrhage.
Usually in the posterior pole.
Resolves in days
Non accidental trauma (NAT)
NOT caused from bouncing a baby, falling off a bed, running with baby.
Signs: Lethargic, vomiting, poor visual response, unequal pupil sizes.
Ocular signs of sexual abuse
Public lice on eyelashes, Chlamydia or gonorrhea,
Corporal punishment
Type of discipline.
Legal in tienesse by teachers and principals.
