abruptio placentae or also known as
PREMATURE SEPARATION OF THE
PLACENTA
It is the premature separation of the normally
implanted placenta after the 20th week of
pregnancy, typically with severe hemorrhage
(ABRUPTIO PLACENTAE)
Etiology
⚫ Multiparity,
⚫ preeclampsia,
separates only at the edges
causing vaginal bleeding & a little pain
MARGINAL ABRUPTION
may not
result in bleeding but increasing uterine irritability &
tenderness
CENTRAL (CONCEALED) ABRUPTION
complete
separation from the uterine wall resulting in profuse
bleeding
COMPLETE SEPARATION (100%)

Assessment
⚫ Sharp, stabbing pain,
⚫ if the placenta
separates first at the
center, blood pools
under the placenta & is
hidden from view;
⚫ blood then may
infiltrate uterine
muscles forming a
hard, board like uterus
with no apparent
bleeding
COUVELAIRE uterus
(UTEROPLACENTAL
APOPLEXY)
Clinical diagnosis
symptomatic
detects bleeding
Ultrasound
decrease normal fibrinogen
results in the absence of normal blood coagulation
Hypofibrinogenemia:
Complications
⚫ Hemorrhagic shock
⚫ Infection
⚫ Prematurity
