if >3cm - vascular referral
if >5cm - urgent vascular referral

consider if >70yrs with known risk factors
presentation
picked up on screening rupture non-specific abdo pain pulsatile + expansile palpable abdo mass incidental finding on X-ray, USS, CT
diagnosing
USS
CT angiogram
classification: normal
< 3cm
classification: small aneurysm
3-4.4cm
classification: medium aneurysm
4.5-5.4cm
classification: large aneurysm
> 5.5cm
Mx - reducing risk of progression
stop smoking
healthy diet
exercise
optiimse htn, diabetes, high cholesterol
Mx- follow up scans
yearly if 3-4.4cm
3mo if 4.5-5.4cm
Mx - elective repair if any of …
symptomatic aneurysm
diameter growing >1cm each year
diameter >5.5cm
DVLA guidance
- inform DVLA if aneurysm >6cm
- stop driving if above 6.5cm
- stricter if drive lorry, bus etc
ruptured AA: presentation
- severe abdo pain radiating to back/ groin
- haemodynamic instability
- pulsative,expansile abdo mass
- collapse
- LOS
