When the plan fails to achieve its intended outcome
Active error is
Error has immediate effect
Latent error is
A previous error has effect in the future
What is a Slip
Actions not carried out as intended / planned
What is a lapse
An Action is missed / forgotten

Error because of bad planning / intentions
Skill based behaviour is;
Routines, muscle memory
Rule Based behaviour is;
A method / procedure learned off
Knowledge based behaviour is;
no established procedure, experience and gen is used
What are the two groups of Recognition Failures
1- Misidentification
2- Non detection
What does R.O.S.E stand for?
R = Routine Violations
O = Optimising Violations
S = Situational Violations
E = Exceptional Violations
What are some factors that promote violations
- time
- Poor equipment

Avoiding extra effort
