8- Cardiac Survival Flashcards
5 step chain of survival
1- Recognition and EMS activation 2- Early CPR 3- Rapid Defib 4- Effective ALS 5- Integrated post-cardiac arrest care
Building blocks of CPR from ok to great
- Chest compressions
- 30:2 CPR
- Multi-rescuer coordinated CPR
What is the best treatment for Vfib
Defib
What is the goal of CPR
To prolong Vfib to increase the chance of survival
Indicator for a Precordial Thump
Witness Vfib without defib immediately available
From the elbow with the fist
When to defib 1st vs CPR 1st
Defib 1st- Witnessed arrest, CPR until defib is ready
CPR 1st- Unwitnessed arrest, 5 cycle of 30:2
What is the GOLD standard for CPR
CPR with ONE defib
What is transthoracic impedance
The energy needed to defibrillate across the body
Indications of synchronized cardioversion
ALL MUST APPLY
- TachyC (wide or narrow)
- Organized QRS
- Pulses present
- Hemodynamically unstable
- ALOC, Ongoing CP, HypoT
Contraindications of cardioversion
- Junctional Tach
- Multifocal atrial tach
- Vfib/VTach
SVT cardioversion Joules
Afib- 100-120 J BiP
AFlutter- 50-100 J BiP
VTach cardioversion Joules
Monomorphic- 100 > 200 > 300 > 360 J BiP, Increase dosage if you want
Polymorphic- Treat as Vfib
