7 - SHOULDER TRAUMATIC/BONY Flashcards
Hill-Sachs lesion
compression fracture of humeral head on posterolateral aspect, notch
What causes a hill-sachs lesion?
Traumatic anterior dislocations / anterior instability
What causes a reverse Hill-Sachs
Posterior dislocation
Bankart lesion
damage to the anterior labrum with a dislocation
Anterior dislocation MOI
Forced external rotation/abduction
FOOSH
Ant.dislocation: signs and symptoms
Arm held in ER/ABD
Restricted ROM
Altered shoulder contour
Bony bankart lesion
Fracture of the anterior glenoid rim
What region is most likely for a bony bankart lesion?
3:00-6:00
Posterior dislocation MOI
MOI: impact seizure/electric shock, arm is in flexion/adduction and force is taken on the hand which pushes the humerus out posteriorly
Posterior dislocation S/S
Elbow held at side with hand on stomach
Can’t ER or Abduct
Easily missed on X-ray
Clavicle fracture clinical presentation
Middle 1/3 shaft typically fractured
Pain +++ on palpation/pulling
Localized tenderness and swelling
Loss of function
Spasm of trapezius and SCM
Arm held to body with shoulder elevated
MOI
(any force that brings the shoulder to midline)
FOOSH
Fall on point of shoulder
Direct superior/anterior force
