what is sepsis?
an infection that can lead to a life threatening organ infection due a disregulated host response
what are the categories of the q sofa score?
RR BP GCS
any of these factors increase, mortality rate increases
what are the stages of child sepsis?
Systemic inflammatory response syndrome
sepsis
severe sepsis
what does a child patient need to have to qualify as having systemic inflammatory response syndrome?
two of:
- temp. high or low
- heart rate high
- breathing rate high
- WCC high or low
what does a child patient need to have to qualify as having sepsis?
systemic inflammatory response syndrome
suspected/proven infection
what does a child patient need to have to qualify as having severe sepsis?
organ dysfunction. usually cardiovascular or respiratory.
shock also.
what is treatment of paediatric sepsis based on?
- looks unwell
- clinical/parental concern
- raised EWS
based on suspicion
what are the paediatric sepsis 6?
1) give high glow oxygen
2) obtain IV/IO access and take bloods
3) Give IV/IO antibiotics
4) consider fluid resuscitation based on lactate
5) involve senior clinicians early
6) consider inotropic support early
what antibiotics would you use treat sepsis?
<1 month
cefoTAXime
1-3 months
ceftriaxione
> 3 months
ceftriazone
how do BP, HR AND HR change with age?
HR and RR decrease
BP increase
as we get older
how does sepsis differ from bacteraemia?
bacteraemia = just in blood
have to have disregulated host response and organ dysfunction
how does septic shock differ from hypovolemic shock and what is the clinical significance?
get vasodilation with septic shock
don’t get with hypovolemic shock
significance = different treatment. give vasoconstrictors and ionotropes in septic shock
