Emergency After-Hours Resupply - After emailing EMSupply (group email) with the resupply need, Paramedics will notify the EMS Coordinator via phone at 405-297- 2865 for re-supply when:
Both Fentanyl and Morphine have been depleted; OR
Both Versed and Valium have been depleted.
Should a controlled substance be lost or damaged, the following shall occur:
Notify EMS immediately at 405-297-1315 during normal business hours or at 405-297-2865 after hours.
An EMS Controlled Substance (IR) Incident Report form will be completed by the responsible Lead Paramedic and forwarded to EMSTraining@okc.gov immediately. The form may be obtained on FDWEB. Damaged or broken containers will be attached to the IR form unless they contain no medication or are a sharps hazard.
An entry will also be made in the Controlled Substance Log indicating the new balance.
EMS will notify the Office of the Medical Director within 12 hours of the incident.
The Medical Director or designee will report the loss/misplacement/theft of the controlled substance, within 24 hours, to the appropriate local civil authorities. State and Federal authorities (DEA and State of Oklahoma Agency for controlled and dangerous drugs) will be notified in writing.
If indicated, an internal investigation will be conducted as a joint effort, by NDO(s) and the Medical Director. This information will be shared, if requested, with local, State, and Federal authorities.
The Office of the Medical Director will conduct a __________ audit of all controlled substances.
