5. Thrombotic Disorders Flashcards
What are the 3 Main Elements of Haemostasis?
- Primary Haemostasis
- Blood Coagulation
- Fibrinolysis
What occurs in Primary Haemostasis?
- Vasoconstriction
- Platelet Adhesion
- Platelet Aggregation
What occurs in Blood Coagulation?
- Insoluble Fibrin Formation
2. Fibrin Cross-Linking
What occurs in Fibrinolysis?
- Urokinase / Tissue Plasminogen Activator (tPA) / Factor XII act on Plasminogen to form Plasmin
- Plasmin acts on Fibrin to form Fibrinogen / Fibrin Degradation PRoducts
What is a Thrombus?
Clot arising in the Wrong Place
What is a Thromboembolism?
Movement of a Clot along a Vessel
What are the Components of Virchow’s Triad?
Thombosis:
- Stasis - Bed Rest / Travel
- Hypercoagulability - Pregnancy / Trauma
- Vessel Damage - Athersclerosis
What are the 3 Types of Thrombosis?
- Arterial
- Venous
- Microvascular
What are the Risk Factors for Arterial Thrombosis?
- Age
- Smoking
- Sedentary Lifestyle
- Hypertension
- Diabetes Mellitus
- Obesity
- Hypercholesterolaemia
What are the Features of an Arterial Thrombus?
- White Clot - Platelets and Fibrin
- Results in Ischaemia / Infarction
- Principally Secondary to Atherosclerosis
What are Examples of Arterial Thromboembolisms?
- Coronary Thrombosis - M.I. / Unstable Angina
- Cerebrovascular Thromboembolism - Stroke / TIA
- Peripheral Embolism - Limb Ischaemia
What is the Management of an Arterial Thrombus?
1. Primary Prevention - Lifestyle Modification / Treatment of Vascular Risk Factors Acute Presentation: 2. Thrombolysis 3. Antiplatelet / Anticoagulant Drugs 4. Secondary Prevention
What are the Risk Factors for a Venous Thrombosis?
Stasis / Hypercoagulability:
- Age
- Pregnancy
- Hormonal Therapy - COCP / HRT
- Tissue Trauma / Surgery
- Obesity / Immobility
- Systemic Disease - Cancer / Myeloproliferative Neoplasms / Autoimmune Disease (IBD / SLE)
- Family History
What are the Features of a Venous Thrombus?
- Red Thrombus - Fibrin and Red Cells
- Results in Back Pressure
- Principally due to Stasis and Hypercoagulability
What are Examples of Venous Thromboembolisms?
- Limb Deep Vein Thrombosis
- Pulmonary Embolism
- Visceral Venous Thrombosis
- Intracranial Venous Thrombosis
- Superficial Thrombophlebitis
