What is the mechanism of action of the combined oral contraceptive pill?

A

Negative feedback by progesterone on the hypothalamus causes reduced secretion of GnRH. This in turn causes reduced secretion of LH and FSH. Low FSH prevent follicular development and secretion of estradiol. Progesterone negative feedback + the absent of estradiol means there is no LH surge. So finally, the lack of follicular development and the LH surge prevents ovulation

It should only be used in patient with known CVD, cerebrovascular disease or other vascular disease
