4. Shoulder Dystocia Flashcards
When is shoulder dystocia identified?
When baby’s shoulders are delayed because the anterior shoulder is stuck behind the symphysis pubis.
Once shoulder dystocia is identified…
Position mother in the McRoberts position.
What quantifies delayed shoulders? (4)
Shoulders not born within two contractions following the head / difficulty delivering face and chin / head remains tightly applied to the vulva or retracts (turtlenecking) / the fetal head fails to fully rotate to face the mothers inner thigh once born.
Descibe McRoberts position.
Mother laid flat with her bottom at the end of the bed and bring her knees to her abdomen. Push with the next contraction.
If the shoulders don’t deliver with McRoberts alone,
use gentle axial traction applied to baby’s head. (Outward traction, keep head in line with spine)
How long should we apply axial traction for?
30 seconds
If undelivered using axial traction, move on to…
Suprapubic pressure still in McRobert’s position.
Suprapubic pressure is…
hands in a CPR grip, heel of hand two finger breeadths above pubis, apply moderate pressure onto baby’s back for 30 seconds.Combine with mother pushing or axial traction
If intermittent pressure is unsuccessful, move to…
ALL FOURS with hips well flexed (like an upside down mcroberts)
Following suprapubic pressure is…
intermittent pressure for 30 seconds.
If axial traction on all fours doesn’t work…
TRANSPORT
__ to the ambulance and position with ___ during transport
Walk, pillow between legs to protect babys head
