33 MI Flashcards
MI
abrupt stoppage of blood
stemi
occlusive thrombi
- results in ST elevation in ECG
- EMERGENCY
to limit infarct size…
artery must be opened w/in 90 min
-PCI or thrombolytic
PCI + thombolytic time frame WITHIN PRESENTATION
cardiac catherization aka PCI- 90 min
thrombolytics- 30 min
classic pain
- heavy pressure, tight, burning, constricted, crushing
- substernal or epigastric
- -may radiate to neck, lower jaw, arms, back
atypical pain
- discomfort
- weak
- nausea
- indigestion
- SOB
diabetic or elderly atypical manifestations
diabetic: silen/asymptomatic
elderly: change in mental status, pulmo edema, sob
SNS stimulation
- during initial phase of MI
- ischemic hrt cells release catecholamine (epineph, norepineph)
- incr HR, BP, vasoconstriction, diaphoresis
bp + hr are high initiall but drop bc
dect CO
-if severe, may result in decr renal perfusion + UO
fever/incr in temp is due to…
systemic inflammatory response to death of myocardial cells
HEART FAILURE
complication that occurs when pumping action is reduced
biomarkers will be __ in NSTEMI but __ in UA
high in NSTEMI
normal in UA
serum levels of troponin (onset, peak, end)
O: 4-6 hr
P: 10-24 hr
E: 10-14 days
serum levels of CK (onset, peak, end)
O: 6 hr
P: 18hr
E: 24-36 hr
2 steps should be completed before thrombolytic therapy
1 draw blood to obtain baseline lab values
2 start 2-3 lines for IV therapy
