MOA

A
  • reduces striated muscle contractions
  • blocks peripheral neuromuscular transmission by reducing acetylcholine release at myoneural junction
  • manages seizures in toxemia of pregnancy
  • induces uterine relaxation
  • can cause bronchodilation after beta agonists and anticholinergics have been administered

A

Seizures of eclampsia
Torsades De Pointes
Hypomagnesaemia
Ventricular fibrillation
Ventricular tachycardia that is refractory to amiodarone
Life threatening dysrhythmias due to digitalis toxicity

Adult dose

A

Seizures of eclampsia:1-4g of 10% solution IV/IO over 3 minutes
Cardiac arrest due to hypomagnesaemia/torsades de pointes: 1-2g of 10% solution IV/IO over 5-20 minutes
Torsades de pointed with pulse: 1-2g in 50-100 ml of D5W (over 5-60 minutes). *follow with 0.5-1g/h IV (tirate to control torsades)

Pediatric dose

A

Pulseless v tach with torsades de pointes: 25-50 mg/kg IV/IO bolus of 10% solution (max 2g)
Torsades de pointes with pulse /hypomagnesaemia: 25-50 mg/kg IV/IO of 10% solution over 10-20 minutes(max 2g)
Status asthmaticus: 25-50 mg/kg IV/IO of 10% solution over 15-30 minutes (max 2g)
