What is Central Serous Retinopathy (CSR)?
Central defect in the retina, retinal detachment with fluid under the retina.
Cause of Central Serous Retinopathy (CSR)?
Unknown
Central Serous Retinopathy (CSR) is unilateral or bilateral?
Unilateral
Why does Central Serous Retinopathy (CSR) have a hyperopic shift?
Elevated cystic area, that shortens the axial length
Symptoms of Central Serous Retinopathy (CSR)?
Blurred vision, distortion, micropsia, positive scotoma- blob.
Central Serous Retinopathy (CSR) OCT and fluorescein angiography results?
OCT: Subretinal fluid, elevated areas, Fluorescein angiography: Leakage of blood vessels (focal leakage), in early stages hyperfluorescent spot, late pooling with dye in subretinal spaces.
What is idiopathic macular hole?
Hole in the macular, the RPE has lipofuscin dye. Usually unilateral, then becomes bilateral.
Cause of idiopathic macular hole?
Unknown. Can arise due to ageing or trauma, myopia, chronic CMO.
Symptoms of idiopathic macular hole?
Variable vision loss, metamorphopsia, jumbling of letters when reading.
OCT of idiopathic macular hole?
Splitting of photoreceptors
Pathogenies of of idiopathic macular hole?
- Early localized perifoveal vitreous detachment.
- Static and dynamic traction
- Open up a hole.
Treatment of idiopathic macular hole?
First stage is to wait, when VA is <6/18 and its been 1 year- vitrectomy
What is epiretinal membrane?
Scar infront of the macular (idiopathic paramacular fibrosis), caused due to posterior vitreous detachment.
Symptoms of epiretinal membrane?
- Reduced VA
- Distortion
- Binocular diplopia
- Macropsia
Signs of epiretinal membrane?
- Glistening
- Crinkled sheen
- Contracture
Treatment of epiretinal membrane?
None if asymptomatic, or else pars plana vitrectomy
Pathogenesis of epiretinal membrane?
Idiopathic, after posterior vitreous detachment. Can be associated with: retinal vascular disease, venous occlusion, diabetic retinopathy, uveitis, trauma, retinal detachment.
What is cystoid macular oedema?
Cystic spaces filled with fluid or oedema at the macular.
Cause of cystoid macular oedema?
- Retinal vein occlusion
- Diabetic retinopathy
- Post cataract surgery
- Chronic uveitis
- Retinitis pigmentosa
Pathogenesis of cystoid macular oedema?
Accumulation of fluid on outer plexiform later, disrupting blood-retinal barrier. Vitreous traction may be involved.
Cystoid macular oedema on FFA & OCT?
FFA: Petaloid appearance
OCT: Lacuna, fluid filled microcysts, bubbles on OCT.
Signs and symptoms of cystoid macular oedema?
Blur or distortion, hyperopic shift, on fundus CL: Honeycomb pattern of retinal thickness, loss of normal foveal reflex.
Treatment of cystoid macular oedema?
Prevention, tropical NSAIDS and steroids to treat underlying uveitis.
