Whipple’s disease
A
MALABSORPTION + lymphadenopathy + arthritis + fever + neuro sx
why malabstoption? → impaired lymphatics due to blockage of enterocytes by lipid laden macrophages
- migratory arthralgias
- weight loss
- abd pain
- diarhhea
- ophtalmoplegia, nystagmus, myoclonus
cause: Tropheryni whipplei
dx: small bowel biopsy → macrophages staining PAS+
rx: antibiotics

abetalipoproteinemia
A
pediatric issue
autosomal recessive
GI tract cells lack ability to make apoprotein B (used to build membranes of chylomicrons and lipoproteins)
can’t absorbe fat-soluble vitamins! → deficiency in ADEK
- ataxia
- retinitis pigmentosa
- spinocerebellar degeneration

A
eosinophilia…
- NOT assoc with allergy or other cause (parasite, etc)
- NOT present in any other organ
abd pain, n/v, diarrhea, malab, wt loss
biospy: 20+ eosinophils/HPF
