Ways of facilitating increased crown height
Surgical crown lengthening whilst maintaining the existing occlusal vertical dimension-
Changing the jaw relationship whilst maintaining present occlusal vertical dimension-
Changing the jaw relationship and increasing occlusal vertical dimension
Short clinical crown height can cause what problems:
Lack of resistance and retention
Poor aesthetics
Solution- can be crown lengthening with or without ortho treatment to allow more over jet
How much is usually a difference between ICP and RCP
Around 1-1.5 mm, usually not more than that
Sometimes, RCP is more posterior to the ICP allowing some space in the anterior region allowing space for restoration
When mounted, if occlusal contact on th retruded arc of closure are eliminated, it will deflect the mandible into a more anterior path of closure
Does tooth wear leads to loss of occlusal vertical dimension
Not if opposing teeth make contact and that is mostly due to continuing eruption of the teeth
So, teeth continue to erupt and bring the alveolar bone and gingival tissues with them leading to an increased display of soft tissue
- In the absence of significant tooth wear, facial height increases throughout life (distance from the nasion to the chin increases)
What is another problem with tooth wear
Loss of occlusal stability
How is eruption/intrusion different in different age groups
In younger pt- eruption is the dominant movement
In older pt- less eruption and more intrusion as there is a gradual loss of eruption potential; takes longer for reastablishment of occlusion
Dahl - mainly for erosive wear but not for bruxism and parafunction
