2nd Page Flashcards
What is Phil’s load rating?
2750 times but a Crane can lift a whole lot more than its max engineering load, but if you are lifting over the load rating, you need to request permission from the city.
Who did you ask to write to the city for permission
Emerson Turnkin
Did you ever get get a response from the city
No
When was the last time you inspected Phill before the August lifts
In April we idled Phil and then expected for metal fatigue. We also replaced all worn and weakened parts
What would happen if the hurricane came before the roof and panels worms
Months of work would be lost
What time did you arrive on the site on August 31st, 2012
A little before 7 AM
What did you do when you got to the site
I walked the site, looked Phill over, and went through my pre-lift checklist.
Why did the foreman raise the red flag at around 8 AM
The crew had found a wrench in Phil’s gears
Was the wrench in Phil’s gears before the lift started
Yes
Did the crew do a safety sweep after they found the wrench
Yes, the safety sweep that proceeded the discovery of the wrench took over an hour
What time did the first panel get lifted
Around 10 AM
When did the first panel get lowered
1:30 PM
After the first lift, what was the weather like
It was gradually getting worse.
Was anyone ever concerned or was anyone ever uncomfortable about lifting in those conditions?
Yes, around 2:15 PM Grace gave me a call and asked me if it was safe to lift with the wind and revised weather report
How did you respond to Grace
I told her that the wind and the load were both in Phil’s 100% tolerance
Did Grace have the authority to shut down the lifts
Yes
Did Grace ever shut down the lifts
No
Was the red flag ever waved during the first lifts
Yes, at around 3:10 PM, the workers found that a fire axe was missing.
Who gave you the green flag after the second race of the red flag?
I am mostly sure that it was Grace, but I was focused on the roof.
What happened after you hit the highest point of the lift
The low hydraulic warning light came on. As I was reaching for the back up hydraulics. The strut snapped. I slammed the airbag button as fast as I could. And the next thing I remember I was in the ER
Why do you think that the crane collapsed
At first I had thought it was just the force of the wind. But if the squatter with the fire axe had chopped the hydraulic lines, that could have caused a collapse too
