25) Drugs Affecting Acid Secretion Flashcards
What are the defensive factors affecting the integrity of the gastric mucosa?
Epithelial integrity
Cell replication
Mucus
Vascular supply
What are the aggressive factors affecting the integrity of the gastric mucosa?
Acid
H.pylori
Drugs
What substances stimulate acid secretion?
ACh, histamine, gastrin
What substances inhibit acid secretion?
Prostaglandin E2, secretin, glucagon, somatostatin
When are antacids indicated?
GORD and ulcers
What are the side effects of antacids?
Diarrhoea, cramps, N&V
What is the mechanism of action of alginates?
Adhere to exposed mucosa and form a raft over the mucosa
Give examples of H2 receptor antagonists:
Cimetidine, ranitidine
When are H2 receptor antagonists indicated?
Ulcers, reflux oesophagitis, functional dyspepsia
What are the side effects of H2 receptor antagonists?
Diarrhoea, headache, gynaecomastia (cimetidine)
When are PPIs indicated?
Ulcers, H. pylori eradication, GORD, dyspepsia, Zollinger-Ellison
What are the side effects of PPIs?
Abdo pain, constipation, flatulence, headache, arthralgia, N&V, diarrhoea
What lifestyle advice can be given to people with GORD?
Eat smaller, more frequent meals Avoid alcohol Stop smoking raise head in bed Maintain healthy weight
What pharmacological treatment can be given to people with GORD?
Antacids, H2 receptor antagonists/PPIs
Why do PPIs have a delayed onset of action?
Only bind to active proton pumps
