For a Level 2 or 3 Investigation, The DESK OFFICER, is required to Notify:

I.A.B & Obtain Log Number
A T.R.I. Investigating Supervisors Assessment Report (Done for Level 2 & 3 uses if force) will be forwarded to:
- First Deputy Commissioner
And additional copies as follows:
- Chief Of Department, (through Channels)
- Deputy Commissioner, Internal Affairs
- Deputy Commissioner, Legal Matters
- Deputy Commissioner, Training
- CO, Risk Management Bureau
- CO, Office of Management Analysis and Planning
- CO of the Members Involved !
UMOS Discharges firearm outside of NYC, he must notify:
Operations, and Local Police Authorities personally or by responsible messenger.
After a UMOS Discharges His firearm (221-04 Firearms Discharge by UMOS). Without waiting for details, the DESK OFFICER will notify:
CO/XO Operations Unit Patrol Borough Command IAB, CC Precinct Det. Squad
When involved in or responding to the scene of a police involved firearms discharge which results in INJURY to or DEATH of a person, ON or OFF duty, within NYC, the PATROL SUPERVISOR Notifies:
-Operations Unit
Request response of:
- IAB Duty Captain
- IAB Personnel
- Patrol Services Bureau Duty Inspector
- Duty Captain
When a person dies, or sustains a SPI and is LIKELY to die In Police Custody or in connection with Police Action the DESK OFFICER will notify:
- Patrol Borough Command
- Operations Unit
- IAB, Command Center
- Detective Squad
- CO/XO, if available.
When, in the course of an investigation (i.e, investigate DOA), a member of the Detective Bureau becomes aware that a death or serious injury was or may have been in connection with a police action, notifications will be made to:
- CO/Duty Captain
- IAB, CC
- F.I.D.
When a MOS is subjected to force while performing lawful duty, or to injury while attempting to apprehend a subject or control an individual or individuals, either on or off duty, the IMMEDIATE SUPERVISOR will notify:
-Precinct Detective Squad to respond if:
- Any outstanding suspects
- Further investigation is required
If the ADA REFUSES to charge the defendant as determined by the investigation, NOTIFY THE:
-CO/XO/Duty Captain
Precinct School Safety Sergeant/Patrol Supervisor will notify who, when a MOS (School Safety or Traffic Agent) injured requires hospital treatment.
Operations Unit
When School Safety or Traffic Agent is hurt/assaulted etc. in the line of Duty, the DESK OFFICER will notify:
- Employee Resources Section
- Operations Unit
- School Safety Division
Inner Perimeter: Only the following are allowed:
- Patrol Borough Commander/Duty Chief
- CO, SOD and personnel under him
- CO, ESU and people under him
- HNT Coordinator and personnel under him
- TARU Supervisor and personnel under him
- Bomb Squad supervisor and 1 Technician WHEN BOMB IS SUSPECTED**
- Ranking Patrol Duty Supervisor from affected operational bureau (Housing, Patrol, Transit) depending on location of incident.
- ANY OTHER PERSON WITH PERMISSION OF THE PATROL BOROUGH COMMANDER OR DUTY CHIEF.
When a UMOS Responds to Hostage/Barricaded Person, they request:
- Patrol Supervisor
- ESU
- HNT
- TARU
At the scene of a Barricaded/Hostage situation, the request for the CO/Duty Captain is made by:
THE PATROL SUPERVISOR.
When responding to an active shooter. The UMOS will ensure the radio dispatcher has notified:
- Patrol Supervisor
- ESU
- SRG
- CRC
- HNT
- TARU
- Bomb Squad
- EMS
