What are the prescription requirements for CD Schedules 2 and 3?
(for human use)
i..e. extra requirements
-be written in indelible ink, but can be computer generated
-be signed by a prescriber (advanced electronic signature is allowed)
-be dated by the prescriber
-Specify address of prescirber (must be in UK and the prescriber must be registered in the UK)
-dose must be clearly defined
-formulation
-strength of preparation (if more than one strength is available for the medicine)
-total quantity written in BOTH words and figures (liquids must be expressed in millilitres)
-Quantity prescribed
-state “For dental use” if it is issued by a dentist
-where Rx is in instalments, valid instalment directions

Maximum rx-ed quantity of schedule 2, 3, 4 CDs should not exceed 30 days
What technical errors for schedule 2 and 3 CDs can be corrected by the pharmacy?
-contains minor typographical error or spelling mistake

-whether either the words or figures (but not both) of total quantity has been missed off
What is needed by the pharmacist in records to correcting technical errors on Rxs for schedule 2 and 3 CDs
Pharmacist must
a) Have exercised due diligence and be satisfied the Rx is genuine
b) Be satisfied that s/he is acting in accordance with intentions of Rx-er
c) Ammend Rx in ink or inedlibly
- to correct the minor typographical error
- or so that Rx complies with the requirement to contain total quantity and number of dosage units in both words and figures
d) Mark Rx so that the amendment is attributable to a pharmacist, e.g. name, date, signature, GPhC registration number

Rx dispensed in community must

- must only be used for schedules 2 and 3

Where do private rx forms (FP10PCD) go once dispensed?
the prescription must be submitted to the relevant NHS agency (NHS BSA) together with the pharmacy identification code - using FP34PCD
-therefore, should contain no other item than CDs, as pharmacists would otherwise by unable to comply with requirement to keep private Rx’s for 2 years
What are the requirements for Veterinary Rxs for CDs
- do not need to be written on standardised forms or submitted to the relevant NHS agency
- do need to be retained for 5 years
When should CDs schedule 2&3 not be supplied on Rx?
- if ti doesn’t comply with Rx requirements
- if Rx-er’s address is not in the UK
- if supplier does not know the signature and does not think it is genuine
- if the supplier hasn’t taken reasonably sufficient steps to satisfy themselves that the signature is genuine
- before appropriate date on Rx
- 28 days after Rx (includes Schedule 4 CDs)
- in the case of an instalment Rx (FP10MDA), if the first instalment is NOT dispensed within 28 days of the appropriate date
