12.7 Chronic Renal Failure Flashcards
How does uremia affected clotting?
Uremia inhibits platelet adhesion and aggregation
where is EPO made? be specific
Renal peritubular interstitial cells
what heart complication can uremia cause?
percarditis
What complicaiton does uremia have on the brain? What is the clinical sign?
encephalopathy (damage to neurons), with asterixis–tremor of hand when wrist extended
what complication of dialysis increases risk of renal cell carcinoma?
growth of cysts in kidneys on dialysis
Uremia: what symptoms (6)
increased nitrogenous waste products in blood (azotemia):
- nausea
- anorexia
- pericarditis
- platelet dysfunction (uremia inhibits platelet adhesion and aggregation)
- encephalopahty with asterixis
- urea crystal deposition in skin
Chronic renal failure: effect on calcium levels. Why?
hypocalcemia
- decreased 1-OH of Vit D by kidneys, so less abosrption of Ca
- hyperphosphatemia means more phosphate binds free Ca
What skin complication does uremia have?
Urea crystal deposition in skin
what complication often happens to shrunken kidneys that undergo dialysis?
Development of cysts. increase risk for renal cell carcinoma
what happens to blood composition in chronic renal failure?
anemia, from loss of EPO
3 most common causes of end stage renal failure
- DM2
- HTN
- glomerular disease
Chronic renal failure:
clinical presentation (all 8!)
MR CA PUNK
- metabolic acidosis (nephrons unable to remove fixed acid from daily protein catabolism, so put pts on low protein diet)
- renal osteodystrophy (osteitis fibrosa cystica–secondary hyperparathyroidism from hypocalcemia)
- Ca: hypocalcemia–from 1) low 1,25 Vit D b/c kidney provides 1-OH step, and 2) increased phosphate binds Ca
- Anemia–from low EPO
- Phosphate: hyperphosphatemia–nephrons cannot excrete
- Uremia–azotemia
- Na retention–edema, HTN
- K–hyperkalemia, nephrons cannot excrete.
