decision making tool used at all levels by identifying, assessing and managing risks. minimizes risks to acceptable levels
4 principles of ORM
accept risk when benefits outweigh the cost, accept no unnecessary risk, anticipate and manage risk by planning, make risk decisions at the right level
discuss hazard severity codes
CAT 1- may cause death, loss of facility/asset or grave danger to national interest
CAT 2- May cause severe injury, illness, property damage, or degradation to assets.
CAT 3- minor injury, illness, property damage
CAT 4- minimal threat to personnel safety
discuss mishap probability
CAT A- likely to occur immediately or short period of time
CAT B- probably occur in time
CAT C- May occur in time
CAT D- Unlikely to occur
define “hazard severity”
assessment of the worst possible outcome that can occur as a result of a hazard.
define “ Mishap Probability”
the probability that the hazard will result in mishap or loss.
function of safety council committee
purpose is to IDENTIFY, DEFINE, and ASSESS issues and problems. gives recommended corrective measure
1- create and maintain active interest in safety
2-serve as a means of communications regarding safety
3- provide program assistance to CO including proposing policy and program objectives
discuss mishap investigation
investigations of mishaps is to ID causes and preventive actions as well as establishing accurate record keeping to prevent similar future events. JAG is responsible for determining accountability.
the hierarchy or preferred order of control measures for fall hazards
1- elimination, removing the hazard from the workplace
2- Prevention, isolating or separating the hazard from the general work place
3- Engineering controls- if hazard cant be eliminated or removed engineering controls is preferred.
4- Administrative controls- introducing new workplace practices or regs.
5- Personal Protective Systems and Equipment- used after all other control measures are determined not to be practical.
purpose of HAZMAT program
HAZMAT control and management focuses on preventing , minimizing, or eliminating the introduction of HAZMAT.
Explain the maintenance and use of PPE importance
many protective devices through misapplication or improper maintenance can become ineffective w/o the knowledge of the wearer.
1- ensure PPE conforms to OSHA
2-train personnel on proper use
3- ensure PPE is worn properly
4- ensure designated personnel perform inspections
5- proper equipment storage against environmental conditions and contamination.
6-ensure compliance with use of PPE
7- ID non-use, misuse or malfunctions of PPE
Discuss electrical or electronic safety and tag out precautions
all energy sources must be secured prior to working, damaged or unsafe equipment must be locked or tagged out of service. only qualified personnel are designated to perform lockout/tagout.
discuss DANGER and CAUTION tag
Danger- when safety of personnel or equipment damage is at jeopardy.
Caution- used to give temporary special instructions. when performing maintenance you have authorizing officer sign the person tagging out system will hang tag and sign.
