101 - General Safety Fundamentals Flashcards
Define Risk
An expression of possible loss in terms of severity and probability
Define Hazard
A condition with the potential to cause personal injury or death, property damage or mission degradation
What is the concept of ORM
is a decision making tool used by people at all levels to increase operational effectiveness by anticipating and managing hazards and reducing the potential for loss, thereby increasing the probability of a successful mission.
Increases our ability to make informed decisions by
providing the best baseline of knowledge and experience available.
Minimizes risks to acceptable levels, commensurate with mission accomplishment. The amount of risk we will take in war is much greater than that we should be willing to take in peace, but the process is the same.
what are the four principles of ORM
Accept risk when benefits outweigh the cost.
Accept no unnecessary risk.
Anticipate and manage risk by planning.
Make risk decisions at the right level.
what are the five steps or the ORM Process
Identify hazards
Assess hazards
make risk decisions
implement controls
supervise
Define Hazard Severity in relation to the Risk Assessment Code
The hazard severity is an assessment of the worst possible consequence, defined by the degree of injury, occupational illness, or property damage which is likely to occur as a result of a deficiency.
how many categories of risk assessment are there
4
what is a a CAT I assessment code
Cat I - Catastrophic - may cause death or loss of a facility
what is a a CAT II assessment code
Cat II - Critical - may cause sever injury or occupational illness, or major property damage
what is a a CAT III assessment code
Cat III - Marginal - may cause minor injury, occupational illness, or property damage
what is a a CAT IV assessment code
Cat IV - Negligible - probably would not effect personnel safety or health, but nevertheless in violation or NAVSH standards
what is the concept of time critical management (TCRM)
applying ORM at the point of commencing or during execution of a mission or task
what does the A-B-C-D stand for in regards to TCRM
A- Assess the situation
B- Balance resources
C- Communicate to others
D- Do and debrief
what are the responsibilities of the Commanding Officer as applied to safety
ultimate responsibility for safety matters within the unit
what are the responsibilities of the Executive Officer as applied to safety
Administrative matters
what are the responsibilities of the safety Officer as applied to safety
managing the occupational health and saftey program.
who does the safety officer report to for Safety occupational health matters
Commanding officers
who does the safety officer report to for Safety occupational health administrative matters
Executive Officer
what are the responsibilities of the DH, DIVO, and WCS, as applied to safety.
Work center compliance with occupational saftey and Health program to include
- Training
-equip
-reporting
what are the responsibilities of the Safety petty officer as applied to safety
-Perform or supervise the performance of planned maineance as required by the maintenance index page (MIP)
-Inspect division spaces and
-submit hazard reports
-.Serve on the enlisted safety committee
what are the responsibilities of all hands as applied to safety
Cooperation and support
what is the safety council
a group of command represatives who make recommendations for policy in safety matters and analyzes the progress of the overall program
how often does the safety council meet
quarterly
who is part of the safety council
-CO or XO (Chair)
-Safety Officer (recorded)
-safety reps from each Dept.
what is the purpose of a mishap investigation
determine the primary and contributing causes of the mishap, and determine if safeguards need to be implemented
who is responsible for a mishap investigation
Major damage/mishap - CO designee
minor/non disabling - 1st / 2nd line supervisors
what are the basic reporting procedures for reporting an unsafe/unhealthful working condition
commands shall post blank copies of report procedures and forms.
posted in a common area
discuss the purpose of a safety plan
address safety and occupational health requirements and execution of plan throughout the project
what are the components of a safety plan
-Identify Required skills and training
-Project organization
-Project scope sheet
-PPE required for safety
-Activity Hazard analysis
-Definable features of work
-Special licenses and permits
-Safety inspection plan summary (engineering controls)
-Safety Equipment required
-special safety plans
who is primarily responsible for the execution of a safety plan and who is responsible for the monitoring
Project Supervisor
Project Manager
what are parts of the execution and monitoring of a safety plan
-Completion/turnover inspection roles and procedures
-report of unsafe or unhealthy working conditions
-Emergency mishap response plan
-Daily Safety lecture log
-Daily safety inspection
-Safety hazard/deficiency
what are the requirements to properly use and maintain PPE
Activities provide necessary PPE and training of use
when is a ground-fault circuit interruption (GFCI) required
All 120-volt, single-phase, 15- and 20-amp receptacles, not part of the permanent wiring of the structure
when is single hearing protection needed
8 hr exposure to noise at 84dB or impact of 140dB
when is fall protection required
when a fall of 4 feet or more is possible or 5 feet for shipyard operations
when is double hearing protection required
exposure to continuous noise exposure to noise exceeding 104dB or peaks of 185dB
