10. Pharmacology of CNS Stimulants Flashcards
What are the 2 categories of ADHD medications?
- Stimulants (amphetamines)
- Non-stimulants
Types of amphetamines
- Amphetamine (d & l) and dextroamphetamine (methamphetamine)
- Lisdexamfetamine (pro-drug)
- Methylphenidate (d & l) and dexmethylphenidate
Types of non-stimulants
- Atomoxetine
- Guanfacine
- Clonidine
MOA of the stimulants used for ADHD?
- Block presynaptic reuptake of catecholamines and DA.
- Interfere with VMAT
- ↑ in NT release —> NE => DA => 5-HT, each at ↑ dose
Which isomers of AMP/MPH have more CNS activity?
D-isomers of AMP/MPH
What is methylphenidate’s main activity (MOA)?
- Inhibition reuptake of DA and other NT pre-synaptic reuptake
- Does NOT stimulate release of NT’s
What must you always assess for before prescribing stimulants for ADHD?
Structural cardiac abnormalities, to prevent sudden cardiac death, strokes and MI
Other RARE SE of stimulants
- Chemical Leukoderma (w/ Daytrana patch)
- Priapism
- Seizures
Describe what the following abbreviations mean:
- IR
- ER/XR
- LA
- CD
- ODT
- Immediate release
- Extended release
- Long-acting
- Controlled delivery
- Oral disintegrating tablet (ODT)
What cautions are there when using stimulants?
- ↑ anxiety/agitation
- Caution in pts with
- Ineffectively-treated bipolar (mania)/psychosis)
- CV disease and moderate-to-severe HTN
- Glaucoma
- Motor tics/Tourettes
- Poorly-controlled seizures
- Hx of drug abuse
What is the onset of activity for stimulants in most patients?
24 hours
How are stimulants administered?
Stimulants are controlled substances (1 month supply only, no refills, no samples); some states, like MO, allow 3 months of Rxs (future-dating)
What is the duration of action of the IR [amphetamine-based stimulants]?
4 - 6 hours
IR [Amphetamine Stimulants]
- Adderall (Dextroamphetamine/AMP)
- Zenzidi (Dextroamphetamine)
- ProCentra (Dextroamphetamine Sulfate)
- Evekeo (Amphetamine Sulfate)
Which IR amphetamine stimulant comes in a liquid form?
ProCentra (Dextroamphetamine Sulfate)
