Amount/Dose
Verify order to drug guide and safe dose range.
Drug
Verify reason for use (indication) and check the MAR to the order.
Route
Verify MAR to order and patient circumstance.
Documentation
Document medication administration immediately after giving medication to the patient.
Refusal
The patient has the right to refuse the medication. The RN is responsible for educating the patient on why the medication is needed and inform the prescriber if refused. Document refusal.
Patient
Identify patient using two identifiers. (Name and DOB)
Assessment
Assess needed labs and vital signs before giving medications.
TIme
Verify MAR to order and hospital protocol.
Evaluation
Make sure the patient has therapeutic effect of medication(s) without significant side effects.
Education
Inform patient of action, therapeutic, and side effects of medications.
