As care givers we do not truly know the experience of our patients so it’s important that we:
Believe what they say!

This is a useful tool that documents severity of pain, fatigue, nausea, depression, anxiety, drowsiness, appetite, sense of well-being and shortness of breath.
The Edmonton Symptom Assessment System
The initial assessment has 5 steps.
- Initial assess & prioritize comfort measures
- Realistic goals w/ pt and family
- Teaching occurs
- Nurse observes family dynamics
- Work to bring family members together to honor and comfort dying member
