10/22 Thiamine Deficiency Flashcards
storage capacity of B1
18 days
how to measure B1 levels
blood sample, RBC transketolase requires TPP
add TPP or dont, if enzymze activity does not increase, no deficiency
active form of thiamine
Thiamine pyrophosphate
TPP
products of dehydration rxns utilizing TPP
CO2, NADH, R-CoA
four enzymes that require TPP
pyruvate dehydrogenase
a-ketoglutarate dehydrogenase
branched chaing keto acid dehydrogenase
transketolast
where does TPP get the PP
ATP
negative regulators of pyruvate dehydrogenase
Acetyl-CoA
NADH
foods rich in B1
dairy
meat (esp pork)
grains and legumes
fruits/vegs
what lifestyle habit prevents uptake of B1
alcohol prevents enterocytes from taking up thiamine
who is a B1 supplement indicated for
alcoholics pregnant women low cal diets 65+ eating disorders elevated metabolism due to infection, inflammation, cancer, trauma
Wernicke Cardiomyopathy
Beri Beri/B1 deficiency cardiomegaly SOB fatigue in alcoholics: peripheral neuropathy, low reflexes, parasthesia, E deficiency
Tx for wernicke cardiomyopathy
glucose AND B1
wernicke encephalopathy
caused by giving ONLY glucose to a B1 deficient pt in a hospital withouth B1
pt will become agitated, confused, ataxic
wernicke-korsakoff
non-reversible
encephalopahy
impaired memory
Often given a coma cocktain in ED before cause is known
what is a coma cocktail
B1, glucose, naloxone
