    Siento no haberle llamado is for if you are talking to someone directly. If you use LO, then it means you are talking about someone else. You are sorry you didn´t call lo - you or him.
  • Siento que no te haya llamado.
  • Lo siento, no lo llamé.
    I asked, all are fine

I checked, both translations are OK and mean the same thing!
You can use le or lo. Llamar is a verb that can use either. Siento no haberlo llamado.
siento no haberla llamada. It is really a DO so lo is best but when a F then you use la.

I´m sorry I called you. Siento haberte llamado. This is sentir using 2 verbs, 1 subj, use inf * pp which in this case is haber llamado!

Can´t use Siento que no te llamé is WRONG - it means I feel that I didn´t call you.

Siento no llamarle is also correct, I ck´d with Julio.

you can say, pero podría haberte pasado but I think you should just use hubiera construction. Actually looks like you can use either
