What is the refferal patterns and innervation of the diaphragm?
Refferal pain to the shoulder (R)
Innervation from C3 C4 C5
What questions would you ask for pleuritic pain ?
“does shoulder pain increase when you cough, laugh, or take a deep breath?”
“Does shoulder pain change when you lie down?
What questions would you ask a patient for potential cardiac pain ?
“have you ever noticed your shoulder pain increasing with exertion that does not require your shoulder?”
“does your jaw, mouth, or teeth hurt when you have shoulder pain?”
” do you notice sweatering, nausea or chest pain with your shoulder pain?
What questions are appropraite if you suspect gastrointestinal conditions? (screen)
Any questions erlated to eating that causes pain
Have you had nausea vomiting, difficulty swallowing
Most common: Hiatal hernia, peptic ulcers
Kerh’s Sign: L shoulder pain
Questions for screening gynecolgic conditions?
“Have you had an eptopic pregnancy?”
“have you missed your last period?
“Irregular spotting/bleeding?”
Any sharp pain in the area is immediate refferal
What are the cluster findings in an oncologic screen?
Age
Lack of MOI
Unexplained weight loss
Pain free PROM
Palpable mass
What are red flags for cervical Myelopathy?
- Sensory disturbance of hands
- muscle wasting of hand muscles
- Unsteady gait
- Hoffman’s reflex
- Bowel and bladder disturbances
- Multisegmental weakness and/or sensory changes
What are red flags for neoplastic conditions?
- Age > 50
- previous history
- Unexplained weight loss
- constant pain with no relief
- night pain
What are red flags for Upper Cervical Ligamentous instability?
- Occipital headache and numbness
- Severe limitation during neck AROM in all directions
- Signs of cervical myelopathy
What are red flags for Vertebral artery insufficiency?
5 D’s and 3 N’s
What are red flags for Inflammatory or systemic disease?
Temperature > 100F
BP >160/95 mmHg
Resting pulse > 100bpm
Resting respiration > 25
Fatigue
