What is the minimum information required for a legal prescription for a non-controlled drug?
A
- prescriber’s signature - include their initials/ forenames, and surname.
practice address - usually GP often pre-printed.
FP10s also contain a number to identify the prescriber. - date next to signature- date signed but also a date for prescription to not be given until
- Patient’s name, address (title not essential) U12 age/ dob, older patients not required.
- Info about product - apart from some controlled drugs, no legal requirement to provide particular info, but freq, no. tablets, type=handy

The FP10 can be taken to any chemist/pharmacy in England; unless patient has exemption a prescription charge of:
Single charge: £9
3-month PPC : £29.10
12-month PPC: £104
