What is BPH
Enlargement of the peripheral zone of the prostate
What is benign prostatic syndrome
Symptoms due to enlargement of peripheral zone of the prostate
Explain incidence of BPH
Increases with age
What are the two groups of symptoms in BPH
Storage
Voiding
What is a mnemonic to remember storage symptoms

What are the storage symptoms
Frequency
Urgency
Nocturia
Incontinence
What is a mnemonic to remember voiding symptoms
UR-HITS
What are the voiding symptoms
Urinary retention Hesitancy Intermittency Terminal dribbling Stream weak
How will BPH present on DRE
Smooth enlarged prostate rubbery texture
How is BPH investigated
All men with LUTS should be offered: abdominal exam, male genital exam, DRE.
And, urinalysis.
What are 3 indications for PSA in men with suspected BPH
- Abnormal DRE
- Concerns about prostate cancer
- BPH causing obstruction
If men with BPH are troubled by LUTS what should they complete
Urinary frequency-volume chart
What should all men with BPH be asked to complete
International prostate symptom score (IPSS) to assess severity of symptoms
What is first-line for BPH
Finasteride (5a reductase inhibitor)
What lifestyle advice should be offered for BPH
- Reduce caffeine and alcohol
- Void twice in a row
- Bladder training
What is first-line medication for BPH
Finasteride
What is finasteride
5a reductase inhibitor
When is finasteride given
Prostate >30g
PSA >1.4
What is an alternative to finasteride
Tamsulosin
What is the MOA of tamsulosin
a-blocker
When is tamsulosin indicated
Moderate-Severe BPH
When is surgery for BPH indicated
If medical management has failed
What surgery is used most commonly for BPH
Trans-urethral resection prostate (TURP)
What surgery can be used for prostates under 30g
Trans-urethral incision prostate (TUIP)
What is TUVP
Trans-urethral vaporisation of prostate
What is HoLEP
Holmium laser enucleation of prostate
What is used for a large prostate
Retropubic prostatectomy
What can as a complication of TURP
Post TURP syndrome
What causes post-TURP syndrome
Irrigation with large amounts of glycine (which is hypos molar) on operating it enters systemic circulation. Causes hyponatraemia. As glycine is metabolised by the liver it produces ammonia which can cause encephalopathy and visual disturbance.
When does post-TURP syndrome become increased risk
- Prolonged surgery
- More fluid used
- Larger prostate
What can BPH lead to
- Acute urinary retention
- Recurrent UTIs
- Bladder calculi
- Hydronephrosis
Explain relationship between BPH and prostate cancer
BPH does NOT increase risk of bladder cancer
what is a mnemonic to remember complications of TURP
TURP
what are complications of TURP
Turp syndrome
Urethral stricture or UTI
Retrograde Ejaculation
Peforation prostate
