What qualifies as PHI?
health info created/received by healthcare provider, health plan, health care clearinghouse, or EMS billing company
exists in verbal, written, or electronic form
info that identifies the individual / where there is a reasonable basis to believe it could
- relates to past/present/future physical or mental health or condition of an individual
- the provision of health care to an individual
- the past/present/future payment for the provision of health care to an individual
is response information PHI?
response information classified as public record (dispatch time, units dispatched, receiving facility transport location) will remain as public record
Identifying information classified as PHI
- names
- all elements of dates (except age) for dates directly related to an individual
- postal address
- geographic areas smaller than a FC supervisory district
- telephone / fax numbers
- e-mail addresses
- SSN
- medical record numbers
- health plan beneficiary numbers
- ID badge numbers
- VIN/license plates
- URLs and IP addresses
- biometric identifiers (finger/voice prints)
- full face photographic images
- any other unique identifying number, characteristic, or code
