exposure to actual or threatened death, serious injury, or sexual violence (directly, witnessing in others, learning it occurred to someone close)
intrusions
traumatic event is persistently reexperienced (can be all 5 senses)
avoidance
persistent avoidance of stimuli assoc w the trauma
negative alterations in cognition and mood (inability to recall aspects of trauma, persistent and exaggerated negative beliefs about self/others/world, distorted cognitions about the cause or consequences of event, negative emotional state, anhedonia, feeling isolated, inability to experience positive emotions)
Horowitz theory of integration
if over control, get neg alternations in mood and cognitions
if under control, get intrusions
duration of symptoms for PTSD
more than 1 month (prior to 1 month, it’s acute stress disorder)
neurobiology of trauma-three key players that contribute to PTSD
amygdala (hyperactivation in response to trauma)
hippocampus (reduced volumes)
prefrontal cortex (LESS activation)
role of hippocampus
contexualizes fear and regulates it on the basis of the situation
role of prefrontal cortex
prefrontal cortex and amygdala are reciprocally related (when PFC is activated, amygdala is inhibited so it’s harder to express fear)
tx for PTSD
behavioral deconditioning (flooding/implosive therapy, systematic desensitization)
cognitive behavioral tx (was demonstrated in the video)
physician screening tool for PTSD (type I trauma-a single incident)
ask:
- have you experienced any vivid thoughts about accident?
- what do you believe is the cause of the accident?
- are you able to return to the place of the accident?
- what do you think could be done to prevent the accident?
look for symptoms
physician screening tool for type II trauma (multiple or prolonged trauma)
-somatic complaints
-chronic depression or anxiety
-difficulty sleeping and concentrating
-memory problems (chunks of time during childhood that aren’t remembered)
-flashback, nightmares
-startle response
-difficult with food that is unappealing
-problems knowing who to trust
