No, despite the name of the form, a new Initial Assessment will be filled out for each visit, including for this recall.

  3. Select your GPG
  4. Click on the Checkmark Folder at the top of the Complete Treatment Screen

There is a refusal of treatment form, but you can also note it in template notes.
