Zestril/ lisinopril WARNINGS
BLACK BOX: FETAL TOXICITY. Do not use in pregnancy.
Talk to doctor if using salt substitutes containing potassium.

Zestril/lisinopril HOW IT WORKS
Reduces your body’s ability to naturally increase blood pressure, helping to avoid high blood pressure
Zestril/lisinopril mechanism of action
Blocks angiotensin converting enzyme, preventing conversion of angiotensin I to angiotensin II in the Renin-aldosterone-angiotensin System (RAAS).
Zestril/lisinopril contraindications
angioedema
(and hyperkalemia?)
Zestril/lisinopril WARNINGS
BLACKBOX: Pregnancy
Precautions: arterial stenosis, hyperkalemia (because angiotensin II would normally stimulate aldosterone, which would stimulate K+ excretion. If this system is blocked, high potassium would increase even more).
Reduce or d/c diuretics before starting Zestril/lisinopril to prevent severe hypotension after first dose.
Avoid potassium salt substitutes and high-K+ diets.
Avoid potassium-sparing diuretics, potassium supplements.
Zestril/lisinopril NORMAL adult dose range
10mg - 40mg daily
Zestril/lisinopril common side effects
- vertigo
- fatigue
- dizziness
- hypotension/ orthostatic
- Hyperkalemia
Zestril/lisinopril ADVERSE REACTIONS
stroke?
liver or kidney failure?

Onset 1 hour
Peak 6-8 hours
Duration 24h
Zestril/lisinopril nursing considerations
Assess for:
- edema
- daily weight
- dyspnea or wet crackles
- BP & Pulse q4h at start
BLACK PATIENTS (?) should take with diuretics
Zestril/lisinopril PATIENT TEACHING
Do not d/c abruptly.
Rise slowly.
Avoid supplemental potassium.
Report dry cough – indication of pulmonary edema.
