Wrist/Hand Flashcards

1
Q

What condition is likely the cause of these findings?

A
  • Loss of the distal interphalangeal joint spaces of all the digits is accompanied by subchondral sclerosis and osteophytes.
  • Nonuniform loss of joint space is present at the 1stmetacarpal phalangeal joint.
  • Diminished joint space visualized at the proximal interphalangeal joints of the 3rd and 4th digits.

DDX for affecting DIPS →

Q

What condition is likely the cause of these findings?

A

DX: Psoriatic arthritis

  • Distal interphalangeal joints narrowed at 2nd, 3rd, 4th and 5th digits on right and 2nd and 3rd digits on the left with associated small subchondral cysts.
  • Marginal erosions are present at these same joints at the bases of the distal phalanges with associated adjacent periosteal reactions given the mouse ears appearance.

Q

Characteristics of ____ include:

  • Predominate involvement of DIPs and PIPs with sparing of the metacarpal phalangeal and carpal jointso
  • Involvement of all joints in a single digit → known as the ray pattern
  • Asymmetric from side/side and digit/digit
  • Marginal erosions with adjacent periostitis, “mouse ears”
  • Whittling of phalanx, erosion into articular surface “pencil in cup”
  • Can see sclerosis of distal phalange “ivory phalanx”

Q

What condition is likely the cause of these findings?

25
Q

What condition is likely the cause of these findings?

A
  • Well defined lucency is noted in the base of the proximal 5th phalange; Medullary based with mild bony expansion. There is cortical disruption present. Subtle stippled calcification is visualized in the matrix.

DX: Enchondromais the most likely diagnosis with an associated pathological fracture
