A

O: humeral head - medial epicondyle via common flexor tendon.
Ulnar head - medial border of olecranon and upper two-thirds of border of ulna.

I: pisiform, hook of hamate and base of fifth metacarpal

D:

A:flexes and adducts wrist

N: ulnar nerve (C7 - T1)

Flexor digitorum superficialis

A

O: humeroulnar head - medial epicondyle via common flexor tendon, medial part of coronoid process of ulna, ulnar collateral ligament, intermuscular septa.
Radial head- upper two-thirds of anterior border of radius

I: tendons divide and insert into sides of shaft of middle phalanx of second to fifth digits

D:

A: flexes fingers and wrist

N: median (C8,T1)

Flexor digitorum profundus

A

O: medial side of coronoid process of ulna, upper three quarters of anterior and medial surfaces of ulna, interosseous membrane

I: base of palmar surface of distal phalanx of second to fifth digits

D:

A: flexes fingers and wrist

N: medial part - ulnar nerve (C8,T1)
Lateral part - anterior interosseous branch of median nerve (C8,T1)
