First Intention/Primary Union
3 phases
Healing occurs from side to side in a sterile wound
Phase 1: Lag phase or Inflammatory Response Phase
Phase 2: Proliferation Phase
Phase 3: Maturation or Differentiation Phase
Second Intention (Granulation)
Generally occurs in large wounds that cannot be approximated
wide irregulat scar may result in herniation
excessive granulation tissue is sometimes refferred as “proud flesh”
Third Intention/ Delayed Primary Closure
a. traumatic surgical wound is deprided and purposely left open to heal by second intention for approximately 4-6 days.
b. the infection free wound is then closed and allowed to finish the healing process through primary closure.
a wound that heals by contraction, granulation, and connective tissue repair
Complications….
Dehiscence (partial/total separation of tissue after closure)
Evisceration (protrusion of the viscera through edges of totally separated wound)
Hemorrhage (can result in postoperative shock)
Infection (microbial contamination)
Adhesion (abnormal attachment of two surfaces normally separate)
Herniation
Fistula (abnormal tract between to epithelium surfaces)
Complications… Keloid scar
a hypertrophic scar formation, occurs in dark-skinned individuals
Complications… Dead Space
separation of wound layers that have not been closely approximated
