Wound Care ATI (Wound Dressings & Wound Drains Summary) Flashcards
These generally work well for wounds with small amounts of exudate, but they can stick to the wound bed of heavily exudate wounds or expose the wound to the outside environment =
Dry Dressings
What should you do for dressing that is stuck to a wound bed?
Pour some 0.9% sodium chloride over the area to moisten the dressing for easier removal
These are used for pt’s who are prone to skin breakdown from pressure, friction, or moisture, including incontinence and wound drainage =
Barrier Creams
Never use a wet-to-dry wound dressing for-
Clean wounds with granulation
What are Wet-To-Dry wound dressings used for?
To remove exudate from wounds (Debridement), though they are much less used now because there are better options (These are painful when removed)
What are Chemical-Impregnated Wound Dressings?
Goofy name aside, these are impregnated with chemicals or agents to speed up the healing process
These are absorbent and provide a moist healing environment while protecting wounds. These are also beneficial for bony prominences because of the padding that these offer =
Foam Dressings
These promote Hemostasis and are helpful for treating wounds with large amounts of exudate and can be used as packing for deeper wounds =
Alginate Dressings
These are like Alginate Dressings but they don’t mess with Hemostasis. These are used for wounds with necrosis, infection, and a need for a moist healing environment =
Hydrogel Dressings
These provide a moist healing environment, help soften necrotic tissue to speed up debridement. These are helpful for deep wound with exudate and are less helpful for dry wounds =
Wound Fillers
This wound dressing provides no absorption but creates a barrier to the environment. These allow for oxygen exchange and a moist environment to promote healing and Autolytic Debridement. Commonly used for necrotic tissue or for superficial tears =
Transparent Film Dressings
These are used for Autolytic Debridement. They have absorptive capabilities while still maintaining a moist environment. Not helpful for infected wounds, helpful for wounds vulnerable to infection. Difficult to assess without removal =
Hydrocolloid Dressings
Penrose Drains are used as-
Open Drainage Systems
What are the drawbacks of open drainage systems?
Difficult to assess the amount of drainage + Difficult to control the transmission of microbes from both the outside environment and the wound itself
These use compression and suction to remove drainage and collect it in a reservoir =
Closed Drainage Systems
