Mechanical wounds
Describe the kind of trauma that caused the injury. Often open and closed wounds.
Wounds r classified by
How the wound was made, size of the wound , depth, cleanliness
Open wound
Surgical incision, injection, or placement of IV catheter.
Closed wound
Tissue is damaged but no break in the skin.
Abrasions
Loss of superficial cells; result of friction or crushing.
Contusions
Blood vessel trauma; result of blunt force. Disruption of of small vessels of the skin.color determines the age of the contusion.
Lacerations
Tearing of tissue; result of excessive stretching sufficient enough to interrupt the continuity of the skin. This type of wound usually has ragged edges.
Incisions
Cut in tissue; produced by sharp instruments, such as scalpel. Sight has clean edge and will heal with less scaring then laceration.
Avulsions
Tearing away of body parts or structure.
Puncture wounds
Deep piercing or penetration of several layers of tissue result of sharp object or instrument or bullet.
Burn
Tissue injury resulting from excessive exposure to thermal chemical, electrical, or radio active agents that can vary in severity.
Thermal wound
Burn resulting from contact with fire, hot objects or fluids. Cause the protein in cells to coagulate.
Ulcers
Wounds occurring when there is inadequate blood flow to tissue causing the skin to break down or to slough. For example decubitus or pressure ulcers. All are a result of insufficient blood flow to the tissue.
Clean wound
usually made intentionally using sterile technique, manually closed and are well- approximated.
Contaminated wound
Usually caused by unintentional break in the skin. Non sterile object, introducing pathogens into the skin. A wound is considered dirty was it is contaminated.wounds that r open and unprotected can be contaminated.
Bandage
A strip or roll of cloth or other material that can be wound around a part of the body in a variety of ways for multiple purposes.
Binder
A bandage that is made of large pieces of material to fit a specific body part; for example a abdominal binder or breast binder.
Dehiscence
Separation of surgical incision or rupture of wound closure typically an abdomen incision .
Drainage
The removal of fluids from a body cavity, wound, or other source of discharge by one or more methods.
Eviseration
Protrusion of an internal organ through a wound or surgical incision especially in the abdominal wall.
Exudate
Fluid cells or other substances that have been slowly exuded or discharged from cells or blood through small pores or breaks in cell membranes
Granulation
Soft pink fleshy projections consisting of capillaries surrounded by fibrous collagen.
Incision
A cut produced surgically by a shape instrument creating an opening into an organ or space in the body
Infectious process
Condition caused by the invasion of the body by pathogenic microorganisms
Inflammatory response
A tissue reaction to injury
Irrigation
Gentle washing of an area with a stream of solution delivered through an irrigating syringe.
Primary intention
Minimal scarring results, healing begins during inflammatory phase of healing in surgery this is usually during closure of the wound.
Puncture
Stab wound for a drainage system
Purulent
Producing or containing pus; thick, yellow, green, tan or brown
Sanguineous
Composed of or pertaining to blood; bright red, indicates active bleeding
Secondary intentions
A wound must granulate during healing, occurs when skin edges are not close together(approximated) or when pus has formed.
Serosanguineous
Then and red composed of serum and blood. Pail red watery; mixture of serous and sanguineous
Serous
then and watery, composed of the serum portion of blood.clear watery plasma
t-tube
A drainage tube into the duct to maintain a free flow of bile until Edema subsides. The tube drains by gravity into a closed drainage tube.
Tertiary intention
Healing by delayed primary intention leaves a contaminated wound open and closes later after infection is controlled.
Vacuum assisted closure
Wound vac is a device that assists in wound closure by applying localized negative pressure to draw the edges of a wound together.
Wound
Any injury to the bodies tissues involving a break in the skin.
