ORG109- What are the responsibilities of OICs/Supervisors in the event of an accident or injury?

A

OICs/Supervisors are responsible for:
* Completing the FRVSafe initial report or ensuring that the reporting / injured /
affected worker has completed the FRVSafe initial report within 24 hours of an
incident occurring;
* Completing an FRVSafe initial report on behalf of a visitor, contractor,
subcontractor or member of the public who is affected by an incident / near miss
/ hazard;
* Notifying their Commander or Manager if the incident is notifiable as per Appendix
A or if the risk rating is rated ‘high’ or above.

What immediate response should be taken if an incident occurs?

A
  • Eliminate or isolate any remaining hazards at the incident site which may have
    the potential to cause any immediate further harm, if safe to do so;
  • Call 000 (if necessary);
  • Notify Supervisor / Officer in Charge (OIC);
  • Provide assistance as per 6.1.1 (below) if an injury has occurred;
  • Supervisor/OIC must immediately notify the relevant Manager/Commander
    unless the hazard / incident is of a minor nature (e.g. a facility issue requiring
    rectification):
    o The Manager / Commander must be provided with information
    regarding the details of the injured or affected person, the facts of the
    incident and any medical transport and destination details which are
    relevant; and
    o The Manager / Commander must notify the relevant ACFO / Director
    and relevant Health & Safety Representative as soon as reasonably practible.

What are 5 incidents that require notification to Worksafe?

A
  • the death of a person
  • a person requiring medical treatment within 48 hours of exposure to a substance
  • a person requiring immediate treatment as an in-patient in a hospital
  • a person requiring immediate medical treatment for o the amputation of any part of his or her body, a serious head injury,
    a serious eye injury, the separation of his or her skin from an underlying tissue (such as degloving or scalping), electric shock, a spinal injury, the loss of a bodily function, serious lacerations (This includes, but is not limited to, one or more
    deep and/or extensive cuts, tears or wounds to the flesh or tissue. The immediate medical treatment required may include stitching to prevent
    loss of blood and/or other treatment to prevent loss of bodily function
    and/or infection)
  • any other injury to a person or other consequence prescribed by the regulations.
  • any incident that exposes a person in the immediate vicinity to an immediate risk
    to the person’s health or safety through the following the collapse, overturning, failure or malfunction of, or damage to, any plant that the regulations prescribe must not be used unless the plant is licensed or registered;
    the collapse or failure of an excavation or of any shoring supporting an excavation, the collapse or partial collapse of all or part of a building or structure, an implosion, explosion or fire, the escape, spillage or leakage of any substance including dangerous
    goods (within the meaning of the Dangerous Goods Act 1985); or
    the fall or release from a height of any plant, substance or object;
    in relation to a mine:
    ▪ the overturning or collapse of any plant; or
    ▪ the inrush of water, mud or gas; or
    ▪ the interruption of the main system of ventilation; or
  • any other event or circumstance prescribed by the regulations.

What are the OICs responsibilities for Workcover claims?

A

The immediate OIC/ supervisor of the claimant is responsible for:
1) Ensuring the initial FRV Safe Report is completed as soon as practical and
assigning the FRV Safe report to an appropriate investigator.

2) Completing the initial FRV Safe report when a claimant is unable to do so and assigning the FRV Safe report to an appropriate investigator.

3) Reporting to the FRV WorkCover Claims Section any additional information that may assist in making a decision concerning the acceptance of liability.

What is required for resumption of duty after a Workcover injury?

A

1) All staff returning to work following time off for a work related injury are to obtain a WorkCover Certificate of Capacity – Full Clearance for preinjury
duties before resuming their pre-injury work. This can be included in a
certificate authorising time off (eg. unfit for duty 28 and 29 October, fit to resume 30 October).

2) FRV, under it’s regulations, may also request operational staff to attend the Brigade
Medical Services (BMS) to confirm fitness for duty before resuming work
