  • A values based directive: where a patient can outline their values/preferences around what they do and don’t want

  • A Victorian resident
  • An Australian citizen
  • You must have decision-making capacity
  • You must be diagnosed with an incurable disease, illness or medical condition that is advanced and progressive (that will cause death within weeks or months, but not longer than 12 months)

To make a request you must make 3 separate requests; two verbal, one written and witnessed with a minimum of 10 days between the first and final request.
