Week 2- Diagnostic Models in Low Back Pain Flashcards
Kinds of diagnosis
- Patho-anatomic
- Prognostic/Treatment based
Patho-anatomic diagnosis
A definable problem with tissue
Prognostic/Treatment based
Diagnosis is based on thee prognosis or the treatment used to treat the pain
Pattern recognition strategies for diagnosis
- Hypothetico-deductive
- Algorithm
- Exhaustive
LOGICAL REASONING IS KEY
Basic steps of the diagnostic process
- Generate possibilities and relative likelihood/probabilities
- Gather new info to clarify initial diagnosis
- Revise pretest and posttest probailities
Pretest probability
There’s a baseline probability for a given condition pre-testing
Post-test probability
A clinical diagnostic test changes the baseline probability
How to we use these principles in actually treating a patient?
Once we achieve a threshold probability for a condition through exam we move forward with treatment
What is the medical model of disease?
- “Tissue is the issue”
- All disease is explained in terms of physical problems
How much back pain can be diagnosed with a definite pathology?
~15%
What is the biopsychosocial model?
Takes into account the behavior, psychology, culture, etc as well as the biological aspects of pain
