What is gestational age
-calculated from first day of LMP
-includes the two weeks before ovulation/conception
-generally when talking about pregnancy, always talking about gestational age unless talking about teratogen exposure
What is embryonic age
-begins at date of conception
-ex: at 12 weeks gestation, fetus is 10 weeks by embryonic age/development
What weeks make up each trimester and what is considered a full term/normal pregnancy?
-1st trimester: 0-13w6d
-2nd trimester: 14w0d-27w6d
-3rd trimester: 28w0d-end of pregnancy
-Full term delivery: 37w0d-42w0d
What happens at first trimester OB visit?
-typically between 6-12 wks
-complete hx: OB, medical, surgical, family, social
-reviews meds for teratogen exposure, prenatal vit
-physical exam
-draw prenatal labs
-US with pregnancy dating
-outline genetic testing: aneuploidy screening/diagnostic testing, carrier screening
OB labs: what labs conducted and CBC value we care about?
-quantitative hCG in blood
-CBC
-ABO RH type
-RPR screen (syphilis)
-MCV value!!!!
What is looked for in the ultrasound in each of the trimesters?
-1st: viability, pregnancy location, number of fetuses, dating
-2nd: anatomy
-3rd: growth, fetal well-being/BPP, fetal position
3rd trimester tests/to-do list
-gestational diabetes screen: between 24-28 wks, determines how well body is able to metabolize extra sugar
-group beta streptococcus (GBS): 36 wks, rectovaginal swab, treatment with antibiotics in labor if positive
-CBC
-Rhogam if indiciated
-STI testing before delivery
What vaccines are recommended in every pregnancy?
Tdap
COVID-19
Influenza
Gs vs Ps?
-Gravidity: total number of confirmed pregnancy regardless of outcome
-Parity: number of times has given birth to a pregnancy that is at least 20 wks gestation
What does each value stand for?
